Apart from having a title, a conceptual artwork depicting the concept art released shows diagrams Mistress of Pain lists several of her abilities; far - more than any normal monster would typical monsters have.

She looks extraordinarily horrible as some perverted , like a demonic, centaur-like hybrid between humanoid of human and spider demon. The She walks on six spider-like legs look like with pointed metal greaves; two smaller, bladed legs protrude where a spider's head or a human female thighs 's abdomen would be. Her upper body is humanoid, but inhuman in that are crammed down into some metal shoes (she has claws), and two "Frontal Blades" sticking out of her crotch. Most of these parts are uncovered by armour protectionmany sharp teeth. The hind section is She has a soft huge spider's abdomen creating two products: Sticky Silk, with large spinnerets that produce sticky silk which impedes player movement, and Explosive Projectiles as well as explosive projectiles that disperse in a cluster around the player. Around this soft abdomen is a hard bone exoskeleton "ribcage"

 In another concept Another artwork, she's found depicts her hanging by her legs in chains, with manacles attached to the legs themselves. She's more dressedher greaves, wearing a "dress-like" attire, with hints of her two Frontal Blades hidden underneath. Her face is also significantly more and a serene than the mad ghoul-like one she has in the shot with notes on itexpression. Considering her appearance, it's likely that she'll be accompanied with some sort The significance of spider-monsters or spider-inspired ones.  What type of significance these chains have, we'll have to wait and see..this has not been made clear.
