The Handmaidens appear as mummified women who are missing their lower halves. It is rumored that the undead sorceress have their lower halves removed during some ritual to increase their power. To compensate for the lack of mobility, the Handmaidens bodies are strapped to predatory flightless bird skeletons. The birds skeletons are from a species of flightless bird native to the Torajan Jungles to the south and appear to be closely related to the Phorusrhacids, or Terror Birds, of South America that lived during the Cenozoic period.

Mephisto's Handmaidens are fierce in combat, using various elemental spells to wage war. A handmaiden will alternate from fire to frost to electrical spells. The skeleton bird's feet and beak are also nasty weapons. The best way to fight Mephisto's handmaidens is to try to knock off the sorceress from the bird skeleton. The sorceress body is easily dispatched while separated from the bird skeleton. The bird skeleton will continue to fight and is more of a struggle to destroy.
